Replacing a Lost or Stolen Passport
To replace a lost or stolen passport, it is necessary for the applicant to appear in person at the Consulate. The application for a replacement passport should be submitted as soon as possible after discovering the loss or theft.

What is Required to Apply to Replace a Lost or Stolen Passport
Please read the following and follow the instructions carefully. Failure to follow these instructions could result in significant delays in processing your application. Note the following requirements:

The applicant must appear in person.

A photocopy of the missing passport or other evidence of U.S. citizenship (e.g., a birth certificate issued in the United States, a Consular Report of Birth Abroad, a U.S. Certificate of Citizenship, a U.S. Certificate of Naturalization, etc.). If you are traveling and do not have a copy of your passport or other proof of citizenship, you can discuss this with a Consular Officer when you apply.

Proof of identity (e.g., a valid passport from any other country, a valid driver’s license with photograph, a national identify card with photograph, etc.). If you do not have proof of identification with you (because, for example, it was stolen along with your passport), you can discuss this with a Consular Officer when you apply.

Two identical passport photographs. The Consulate cannot accept any photographs that do not meet the specified requirements. For those requirements, follow this link to Passport Photo Specifications.
Make an Appointment or Come to the Consulate?
Replacing a lost or stolen passport still requires an appointment, with certain exceptions as noted below. To make an appointment, following this link to Scheduling an Appointment.

If you are a visitor or temporary resident of Jerusalem, the West Bank, or Gaza and your passport has been lost or stolen, you should come to the Consulate during public hours (Monday through Friday, between 8:15AM and 11:30AM) and complete a “Request for Emergency Services” available from the receptionists at the Consulate entrance. Once your request is accepted, you will receive further instructions.

If you are resident in Israel, you are required to make an appointment for a replacement passport unless you will be traveling immediately to the U.S. for emergency purposes only. If you are claiming an emergency, you will need to complete a “Request for Emergency Services” form available from the receptionists at the Consulate entrance, explaining why your travel should be considered an emergency. You will also be asked to provide evidence of immediate travel or other information to corroborate your situation. If your case is not deemed to be an emergency, you will be asked to make an appointment.
Fees
The fee for a replacement for a lost or stolen passport is $100. This fee may be paid in U.S. dollars (please ensure the bills are not torn) or in Israeli shekels at the current exchange rate. Payment can be made in cash or credit card only. No personal checks are accepted.
Delivery
The Consulate no longer accepts self-addressed, stamped registered mail envelopes for in-person applications. When you come to the Consulate for your appointment, you will need to complete an envelope from one of two contracted courier services. The courier service will either deliver your documents directly to your chosen address, or call you for pick-up at one of their centers.
The delivery time for a passport is generally two to three weeks. Emergency passports are generally available for pick-up the afternoon after the application has been submitted.
