Lost, Stolen, or Mutilated Passports

To facilitate access to the Consular Section, applicants for any service, including American citizens, are requested to carry only those items that are absolutely necessary.  No weapons are permitted at any time.  Electronic items must be left at the reception desk, and this is a time-consuming process that will delay your entry.  Metallic items will cause delays during screening, and all bags must be x-rayed.

What to do if your U.S. Passport is Lost, Stolen or Mutilated

Notice to American Citizens:

If your passport was lost, stolen or mutilated, you must come to the Consulate at 27 Nablus Road to apply for a replacement. You must bring proof of citizenship and identity. Documents accepted include a U.S. birth certificate, Consular Report of Birth Abroad, or Certificate of Naturalization to prove citizenship and a U.S. or foreign driver’s license or a foreign passport or identity card to prove identity. You should bring original documentation. If you cannot meet all of the requirements above, you may wish to have an American friend or relative come with you to the Consulate to serve as an Identifying Witness. This individual must bring his or her U.S. passport.

The Consulate General has implemented an appointment system for American citizen services. This will ensure that an officer is always available to assist our clients and that clients receive services as promptly as possible. Clients applying for Consular Reports of Birth Abroad (CRBAs), passports and notaries are required to book an appointment on our website before coming to the consulate for services. Those without appointments will not be admitted, except on an emergency basis. Please read the following information regarding passport applications and follow the link to make an appointment.

In the case of recently lost or stolen passports with impending travel, please present your police report, other supporting documentation regarding the need for travel and information at the reception booth on Nablus Rd. Your situation will be reviewed by a consul on a case-by-case basis and an emergency passport may be issued without a previously scheduled appointment.

If your passport has been lost/stolen for more than two weeks, please read the below information and schedule an appointment to replace your documents.

The following is required to apply for a replacement U.S. passport:

  • AN APPLICATION FORM
    If you are a first-time applicant, replacing a lost, stolen, or mutilated passport, or renewing an expired passport that was issued more than 15 years ago, you must submit
    Form DS-11.
  • 2 PHOTOGRAPHS
    You must submit 2 recent photographs, taken within the last 6 months. Photos should be two by two inches, front view, on a white background.
  • PASSPORT FEE
    Adults applying for a first time passport, replacing a lost, stolen or mutilated passport, or renewing a passport issued more than 15 years ago must pay $100 U.S. Dollars.

    Children under 16 years of age applying for first time passports, replacing lost, stolen or mutilated passports, or renewing a passport must pay $85 U.S. Dollars.
You may also pay in Israeli shekels at the current exchange rate. Payment can be made in cash or credit card only.

PASSPORT AND REPORT OF BIRTH RETURN:  The Consulate no longer accepts self-addressed, stamped registered mail envelopes for in-person applications.   When you come to the Consulate for your appointment, you will need to complete an envelope from one of two contracted courier services.  The courier service will either deliver your documents directly to your chosen address, or call you for pick-up at one of their centers.

SOCIAL SECURITY NUMBER

Please note that Federal Tax Law requires applicants to enter their social security numbers on their passport applications. If you have never applied for a social security number, enter zeros instead. If you do not know your social security number, you can call the Social Security Administration at 1-800-772-1213 from the USA or 1-880-772-1213 from Israel or the Palestinian Authority.
