Name Changes and Corrections
As of October 2005, the State Department no longer amends or extends valid U.S. passports. U.S. citizens requesting a name change or correction for a passport must apply for replacement passports, rather than an amendment to their current passport. Please note the following:

To correct a passport or change a name on a passport that was issued less than one year ago, the applicant should follow the instructions on this page below.

To correct a passport or change a name on a passport that was issued over one year ago, the applicant must apply for a new passport and pay the appropriate fees. Please follow the link to Passports and follow the appropriate link.
Corrections to Passports Issued Within the Past Year
If the passport was issued within the past year, this service does not require an appointment. Simply come to the Consulate Monday through Friday (except Wednesdays) between 815AM and 1130AM, and explain to the receptionist that you are coming to apply for a change to your passport.
The following are required to make corrections or name changes to a passport:

The applicant’s current, valid passport.

Two identical passport photographs. The Consulate cannot accept photographs that do not meet the specified requirements. For those requirements, follow this link to Passport Photo Specifications.
Fees
There is no fee for this service, provided it is completed within a year of the passport's issuance.
Delivery
The Consulate no longer accepts self-addressed, stamped registered mail envelopes for in-person applications. When you come to the Consulate for your appointment, you will need to complete an envelope from one of two contracted courier services. The courier service will either deliver your documents directly to your chosen address, or call you for pick-up at one of their centers.
The delivery time for a passport is generally two to three weeks.
