Passports

To facilitate access to the Consular Section, applicants for any service, including American citizens, are requested to carry only those items that are absolutely necessary.  No weapons are permitted at any time.  Electronic items must be left at the reception desk, and this is a time-consuming process that will delay your entry.  Metallic items will cause delays during screening, and all bags must be x-rayed.

Special Procedures for Issuing or Renewing Passports for Children Under 16 Years of Age

Before coming to your appointment, please review the instructions on the State Department's website.  

The Consulate General has implemented an appointment system for American citizen services. This will ensure that an officer is always available to assist our clients and that clients receive services as promptly as possible. Beginning August 6, clients applying for Consular Reports of Birth Abroad (CRBAs), passports and notaries will be required to book on appointment on our website before coming to the consulate for services. Those without appointments will not be admitted, except on an emergency basis. Please read the following information regarding passport applications and follow the link to make an appointment.

Notice to American Citizens:

Public Notice to American Citizens Regarding Passport Requirements for Minors: if both parents of a minor passport applicant do not appear at the Consulate in person, the parent who does not appear must provide a notarized consent statement, please see details in #5 below.

In order to apply for a passport for a child under 16, you will need to present the following items:

1. Passport application Form DS-11.  You may obtain one from the Consulate General or you can download it from the Internet.

2. Evidence of your own identity.  Both parents must submit one of the following:

-- a valid U.S. or foreign passport
-- a valid driver’s license
-- a valid U.S. Naturalization certificate
-- a valid local ID card with photo

If only one parent is present, then he/she must also present the identity document of the second parent unless the second parent’s consent is not required.

3. Evidence that your child is a U.S. citizen.  You should bring one of the following documents for the child:

-- a certified U.S. birth certificate
-- a previously issued U.S. passport
-- a Report of Birth Abroad (Form FS-240)
-- a Certification of Birth Abroad (Form DS-1350)
-- a Certificate of Naturalization

Please Note: Only the originals of these documents are acceptable (photocopies are not).  A certified birth certificate must have the registrar’s raised seal and the date the certificate was filed. Birth abstracts are not acceptable.

4. Evidence of your child’s relationship to you. You should bring one of the following documents:

-- a certified birth certificate showing both parents' names
-- a certified foreign birth certificate showing both parents' names, along with a translation if needed
-- a Report of Birth Abroad (Form FS-240) with both parents' names
-- an adoption decree with the adopting parents' names
-- a court order establishing your custody or guardianship of the child

5. Evidence that both parents consent to the passport issuance.  The Bureau of Consular Affairs of the U.S. State Department has announced a change in the rule governing issuance of a U.S. passport to minors under the age of 16. This change is effective worldwide, and is not limited to the Jerusalem consular district.

-- If one parent does not appear in person at the time of the application, that parent must provide a notarized consent form agreeing to the issuance of the passport. The non-appearing parent should use form DS-3053.

-- Execution of application by one parent. The two-parent consent requirement does not apply if the parent appearing in person provides documentary evidence (such as a death certificate or child custody order) that he/she has sole custody of the minor child, or in cases involving compelling/exigent family circumstances.

6. Photographs. Submit two identical color photographs, 2x2 inches, on a white background, taken within the last 6 months. Colored backgrounds (e.g., blue, red, green, etc. will not be acceptable).

7. Fees. For first-time passport applications for children (under 16 years) and to renew a child's passport the fee is $85.

The current exchange rate is $1.00 U.S.D = 3.8 NIS.

PASSPORT AND REPORT OF BIRTH RETURN: To receive your passport, you may either: 1) supply a registered, self-addressed, stamped envelope (for registered mail) or 2) complete an envelope upon arrival at the Consulate General for a courier service that will deliver your documents directly to your chosen address cash on delivery.

8. Social Security Number.  You must provide your child’s social security number (SSN) on the DS-11 form if he/she has one, otherwise the Internal Revenue Service may impose a $500 penalty on you. If your child doesn’t have a SSN, you should enter all zeros on the form, and then contact our Social Security Section for information on obtaining a social security number for your child.
