Passport Renewals for Applicants Under 16 Years Old
All children under the age of 16 must apply in person for passports, whether applying for the first time or for a renewal. In addition, both parents must appear in person with the child. If one of the parents cannot come in person, the parent not appearing must provide a signed and notarized consent form.
Passports for applicants under the age of 16 are valid for five years.
What is Required for the Appointment
Please read the following and follow the instructions carefully. Failure to follow these instructions could result in significant delays in processing your application. Note the following requirements:

The child and both parents must appear in person. If one of the parents cannot come in person, he or she must sign and have notarized a consent form authorizing the passport application without his or her signature and appearance. The child's most recent U.S. passport

Proof of identity of the parents, which can take the form of:

a valid passport from any other country

a valid driver’s license with photograph

a national identify card with photograph

a valid U.S. or State government identity card

a U.S. military identity card

Proof of parentage, which may take the form of the child’s American or foreign birth certificate or Consular Report of Birth. (Please note that the appearance of the child’s name on a foreign-issued identity card is not considered proof of parentage for U.S. government purposes.)

Two identical passport photographs. The Consulate cannot accept any photographs that do not meet the specified requirements. For those requirements, follow this link to Passport Photo Specifications.

Name change documentation (if applicable). If you have changed your child’s name, or one of the parent’s names no longer matches the name as it appears on the proof of parentage, you will be required to furnish an official name-change document issued by the Israeli or Palestinian Authority Ministry of Interior that clearly shows the original name and the new name. In cases where the name changed through marriage, the marriage certificate is sufficient proof.
Fees
The fee for a passport for applicants under the age of 16 is $85. This fee may be paid in U.S. dollars (please ensure the bills are not torn) or in Israeli shekels at the current exchange rate. Payment can be made in cash or credit card only. No personal checks are accepted.
Delivery
The Consulate no longer accepts self-addressed, stamped registered mail envelopes for in-person applications. When you come to the Consulate for your appointment, you will need to complete an envelope from one of two contracted courier services. The courier service will either deliver your documents directly to your chosen address, or call you for pick-up at one of their centers.
The delivery time for a passport is generally two to three weeks.
