Step 2: Notification

To notify the Consulate that your documents are prepared, you should send Form DS-2001 (formerly OF-169), Instructions for Immigrant Visa Applicants, to the Consulate via mail or submit it to the reception desk, Monday through Friday, between 8:30 and 10:30 am. This form must be completed and signed. Once we have received this form, we will schedule you for an appointment. Notification of an appointment will be sent by mail.

  • Do NOT send any other documents.
  • Do NOT request an appointment simply to submit this form.
  • Do NOT call to inquire if your forms have been received.
