Tips for Completing the DS-11 Form
The Application for a U.S. Passport - DS-11 form may be completed by typing the required information into the electronic form. The application should then be printed on white A4 or letter-size paper, with each page on a separate sheet of paper. If choosing to complete the application form by hand, it should be completed in block letters and black ink.
When completing the form, please take note the following tips:
- Item 5 - Social Security Number. You must complete this item. If you have never had a Social Security Number (SSN), write zeros in the boxes. If you have a number, but do not remember it, you may request the number at the Federal Benefits Unit at the Consulate General or fill out a form when you appear at the Consulate General which authorizes the Consulate to look up the number and include it on the form.
- Item 6 - Mailing Address. Complete this item with a local address, either your home address or the address where you are staying. Please note that you may choose to have your new passport sent to a different address when you complete your courier envelope at the Consulate General.

Item 17 - Permanent Address or Residence. Complete this item with any address you consider to be your permanent address.

Items 18 and 19 - Travel Plans and Emergency Contact. This information is not required, but may be helpful in processing your application and in assisting you in the future. Suggested contacts are your spouse, parent, adult children, etc.

Oath and Signature. Please do not sign the form. You are required to sign in the presence of the Consular Officer at the Consulate General. You must read the "Acts or Conditions" as outlined in the instruction pages before you sign. If any mentioned acts or conditions have been performed by or apply to the applicant, cross out only the applicable portion(s) and submit an explanatory signed statement with the application.
